    Please also remember to bring your insurance card, photo identification, and any copays, co-insurance or deductible payments
     Please arrive 15 to 30 minutes early to your first appointment to fill out paper work.

    Appointment Policy

    Missed Appointment our goal is to provide quality individualized care in a timely manner. No-shows, late shows, and cancellations inconvenience those individuals who need access to treatment. 

    We would like to inform you of our policy regarding missed appointments.
    Cancellation of an Appointment-To be respectful of the needs of other patients, please be courteous and call our office promptly minimum of 24 hours in notice if you are unable to show up for an appointment. This time will be reallocated to someone who needs treatment. If it is necessary to cancel your scheduled appointment, I require that you call at least 24 hours in advance.  

    If you fail to do so then you subject to cancellation fee please see Late cancellations and No Shows for more information.   Appointments are in high demand, and your early cancellation will allow another patient access to timely care.

    How to Cancel Your and Reschedule Your Appointment-To cancel your appointment, please call our office.  If you do not reach us you may leave a detailed message. If you would like to reschedule your appointment, please leave your name and phone number with the rescheduled appointment request. We will return your call promptly. (please specify if you would like to be reached after hours)

    Late Cancellations-A cancellation is late when the appointment is cancelled without a 24-hour advance notice and is subject to the cancellation fee which is the full amount of the session.

    No Show Policy:   A “no-show”, is a patient who misses an appointment without cancelling it. A failure to be present at the time of a scheduled appointment will be recorded in the patient’s chart as a “no-show”. This includes arriving 30 minutes after your scheduled appointment. without notification.  You will be subject to the same fee as if you did not cancel our appointment.

     Life does happen and the first-time occurrence there is a “no-show”, or late cancellation with a reasonable excuse there will be no charge to the patient. For other occurrences may also apply but is only done a case by case basis. Most will be subject to the fee for late cancellation or no show


    Our office accepts various types of payment for services he renders. Beginning therapy or seeing a therapist requires a personal commitment. It also requires being responsible financially for the process. We would like you to understand not only your responsibilities but ours as well.

    • We are currently in the process of negotiating contracts and will continually assess our contracting options to provide the best possible care for our clients and families.
    • Out of Network**If your insurance does not list your me as an in-network provider, your services may be considered "out-of-network" or not be covered and you may be responsible for all or a larger portion of your bill.​ If you have out of network benefits you may be able to file with your insurance for reimbursement.
    • Although I do verify your eligibility please feel free to verify your health insurance coverage when you arrange your first visit f or your understanding. of what is covered.
    • If you do not see your insurance here, please  call and ask me. Insurance and EAP  panels change and are not always updated regularly.

    Private  Pay

    Self Pay
    Patient elects to reimburse for services  rendered by cash payment. Payment is expected at the time of service.
    Payment Assistance
    Based on your household income, you may qualify for partial financial assistance, on a sliding scale. Sliding-scale fees based on income, are available for the uninsured. These agreements are subject to proof of income. If you qualify for a sliding scale and have no payment plan, please pay the full sliding scale fee before your session.
